We are deeply invested in our core value to put patients first, and also deeply committed to creating a culture where every employee is valued and we learn and improve together. </p> <p><strong>About the Role:</strong></p> <p>We are seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity, and to collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care. Work is remote, and schedules can be flexible. </p> <p>We are currently accepting applications for a<strong> Modified FTE (32hrs/week) Obesity Medicine Physician</strong> opportunity, who are licensed in <strong>AL, TN, OK, MO, IN, SC or GA.</strong></p> <p>This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.</p> <p>This is a Modified Full Time position based in a remote, telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for administrative duties. </p> <h3><strong>What you will do:</strong></h3> <ul> <li>Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management. The physician will have their own panel and will collaborate with APP colleagues and Registered Dietitians.</li> <li>Review charts and conduct <strong>return visits for APP patients every third visit</strong> to ensure quality and continuity of care.</li> <li>Provide <strong>treatment planning and medical management support</strong> for complex cases or escalated concerns.</li> <li>Prescribe <strong>non-controlled obesity medications</strong> for patients under both direct care and in coordination with APPs.</li> <li>Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.</li> <li>Assist in the <strong>professional development</strong> of APPs by fostering clinical growth, offering feedback, and supporting continuing education.
