Position Overview

The Moments Hospice Triage RN plays a critical role in supporting patients and families after normal business hours. This nurse serves as the first point of contact, providing clinical guidance, emotional support, and coordination of care using the nursing process and Telecare protocols.

This role blends live phone triage with clinical workflow: when not on a call, the nurse processes Home Care Home Base (HCHB) workflow and completes documentation and follow-up. Both call handling and workflow completion are core, measured responsibilities - the quieter call periods are productive working time, not downtime.

This is a fully remote position ideal for an experienced hospice nurse who thrives in a fast-paced, patient-centered environment.

What You'll Do

  • Provide after-hours clinical assessment, triage, and interventions via phone
  • Process HCHB workflow and complete documentation/follow-up between calls - a measured part of the role, not downtime
  • Serve as an advocate for patients and families by coordinating with physicians, facilities, and care teams
  • Coordinate care delivery, including dispatching visits and collaborating with vendors
  • Deliver patient/family education and provide compassionate end-of-life support
  • Document all interactions, assessments, and care plans accurately in the EMR
  • Review documentation from team members for compliance and quality standards
  • Support Telecare staff to ensure optimal patient outcomes
  • Participate in training, team meetings, and ongoing professional development

What We're Looking For

Required:

  • Active RN license (no restrictions)
  • Compact RN License or Licensed in the states Moments Hospice operates
  • Prior triage experience
  • Hospice experience
  • Home Care Home Base (HCHB) experience

Preferred:

  • Compact RN license
  • Call center or additional triage experience
  • 2+ years in acute care, hospice, home health, oncology, or ER

Skills:

  • Strong clinical judgment and critical thinking
  • Excellent phone communication and de-escalation skills
  • Ability to multitask and work independently in a remote setting
  • Comfortable navigating multiple systems while on calls
